目的 探究护士应用糖尿病患者出院准备服务证据的障碍因素，为制订契合临床情境的循证护理实践方案提供参考。方法 采用目的抽样，于2024年1-2月选取湖南省某三级甲等医院代谢内分泌科室的16名临床护士进行半结构访谈。依据理论域框架拟订访谈提纲，并运用内容分析法对资料进行分析。结果 从护士应用糖尿病患者出院准备服务证据的障碍因素中提炼出2个主题：护士个体层面的证据应用内在障碍，组织与情境层面的证据转化外在障碍。结论 护理人员在开展证据转化时，需结合证据转化场所的实际情况，对证据进行裁剪，以制订针对性的循证实践方案，提升糖尿病患者的出院质量。

Objective To explore the barriers encountered by nurses in applying evidence-based discharge preparation services for patients with diabetes,and to provide a reference for developing context-specific evidence-based nursing practice protocols.Methods Purposive sampling was used to select 16 clinical nurses from the Department of Metabolism and Endocrinology at a tertiary A hospital in Hunan province for semi-structured interviews from January to February 2024.The interview outline was developed based on the theoretical domains framework (TDF),and data were analyzed using content analysis.Results Two main themes were identified regarding the barriers:internal barriers to evidence application at the individual nurse level,and external barriers to evidence translation at the organizational and contextual level.Conclusions When implementing evidence translation,it is necessary to tailor the evidences according to the actual conditions of the clinical setting to develop targeted evidence-based practice protocols,thereby improving the quality of discharge preparation for patients with diabetes.
