目的 探讨辅助生殖技术(assisted reproductive technology,ART)助孕女性经济毒性的现状及其影响因素,为制订针对性干预措施提供依据。方法 2024年9-12月,采用便利抽样法选取湖南、广西两省3所三级甲等医院生殖中心就诊的314例ART助孕女性为研究对象,采用患者报告结局经济毒性综合评分量表(comprehensive scores for financial toxicity based on the patient-reported outcome measure,COST-PROM)、中文版不孕症生育压力简式量表(Chinese version of the COMPI fertility problem stress scale,COMPI-FPSS)、感知伴侣回应性量表(perceived partner responsiveness scale,PPRS)、心理弹性量表(Connor-Davidson resilience scale,CD-RISC) 对其进行调查。结果 ART助孕女性COST-PROM得分为(15.72±7.55)分,其中92.04%(289例)存在经济毒性。多元线性回归分析显示,门诊报销比例、家庭储蓄、助孕次数、异地医疗、ART相关并发症、生育压力、心理弹性是经济毒性的影响因素(均P<0.01),解释总变异的41.74%。结论 ART助孕女性普遍存在经济毒性,医护人员应早期识别高危人群,并根据影响因素制订针对性生育护理策略,以降低该人群的经济毒性水平。

Objective To explore the current status and influencing factors of financial toxicity among female patients undergoing assisted reproductive technology (ART),and to provide a basis for formulating targeted intervention strategies.Methods Convenience sampling method was employed to select 314 female patients undergoing ART treatment in three tertiary A hospitals located in Hunan and Guangxi provinces from September to December 2024.Comprehensive Scores for Financial Toxicity Based on the Patient-Reported Outcome Measure(COST-PROM),Chinese Version of the COMPI Fertility Problem Stress Scale(COMPI-FPSS),Perceived Partner Responsiveness Scale(PPRS),and Connor Davidson Resilience Scale(CD-RISC)were used for investigation.Results The COST-PROM score was 15.72±7.55,with 92.04% (n=289) experiencing financial toxicity.The multiple linear regression analysis showed that outpatient reimbursement ratio,family savings,number of assisted pregnancies,remote medical treatment,ART-related complications,fertility pressure and psychological resilience were the main influencing factors of financial toxicity (all P<0.01),explaining 41.74% of the total variation.Conclusions Financial toxicity is prevalent among ART-treated women.Healthcare providers should identify high-risk groups early and formulate tailored reproductive care strategies according to influencing factors to reduce the level of financial toxicity.
