目的 通过对前期1项随机对照试验数据进行二次分析,评价迷你中长导管和中等长度静脉导管在减重手术患者围术期的经济性和有效性。方法 2022年8-12月,选取在浙江省某三级甲等医院进行减重手术的124例住院患者为研究对象,采用TreeAge Pro软件构建迷你中长导管和中等长度静脉导管在减重手术患者围术期应用的决策树模型,并通过敏感性分析验证结果的稳健性。结果 中等长度静脉导管组相比迷你中长导管组的增量成本效果比为2868.51元。单因素敏感性分析结果显示,迷你中长导管和中等长度静脉导管的并发症发生率对增量成本效果比值影响较大;概率性敏感性分析显示,中等长度静脉导管相对于迷你中长导管具有经济性概率为62.3%。结论 假定以2022年中国人均可支配收入为患者的个人意愿支付价格,减重手术患者围术期使用中等长度静脉导管比迷你中长导管更具经济性。

Objective A secondary analysis of data was conducted from a previous randomized controlled trial to evaluate the cost-effectiveness of mini-midline versus midline catheters in bariatric surgery patients during the perioperative period.Methods From August to December 2022,124 hospitalized patients undergoing bariatric surgery at a tertiary A hospital in Zhejiang province were selected.A decision tree model was constructed using TreeAge Pro software to compare mini-midline and midline catheters,with sensitivity analyses performed to verify result robustness.Results The incremental cost-effectiveness ratio(ICER) for midline catheters versus mini-midline catheters was 2,868.51 CNY.Univariate sensitivity analysis revealed that complication rates significantly influenced ICER values.Probabilistic sensitivity analysis showed a 62.3% probability of midline catheters being more cost-effective.Conclusions With the 2022 per capita disposable income in China as the willingness-to-pay threshold,midline catheters demonstrate better cost-effectiveness than mini-midline ones for perioperative use in bariatric surgery patients.
