总结1例采用单管双腔插管实施静脉-静脉体外膜肺氧合（veno-venous extracorporeal membrane oxygenation,VV-ECMO）救治的重症肺炎患者的护理经验。主要护理包括：食道超声心动图引导下精准置管的护理配合；动态评估双腔插管位置观察再循环，确保ECMO有效氧合效率；预防院内感染，强化免疫抑制状态下的感染防控护理；设置个体化抗凝目标，预防插管相关性深静脉血栓形成；呼吸力学指导下实施清醒ECMO；多学科团队协作，助力早期康复。经过精心治疗和护理，患者入院第20天成功撤离ECMO，第36天转入普通病房，病情平稳。

To summarize the nursing experience in a case of a patient with severe pneumonia who was treated with veno-venous extracorporeal membrane oxygenation(VV-ECMO) using a single-lumen double-cuffed endotracheal tube.The nursing care mainly included the following aspects:cooperating with the medical team to achieve precise cannulation under the guidance of transesophageal echocardiography (TEE); dynamically assessing the position of the double-cuffed catheter and monitoring recirculation to ensure effective oxygenation by ECMO; focusing on infection prevention and control, especially in the context of the patient’s immunosuppressed state; establishing individualized anticoagulation targets to prevent catheter-related deep vein thrombosis (DVT); implementing awake ECMO based on respiratory mechanics guidance; and facilitating early rehabilitation through multidisciplinary collaboration. Following comprehensive treatment and nursing care, the patient successfully weaned off ECMO on the 20th day of hospitalization and was transferred to a general ward on the 36th day with stable condition.
