[摘要]
目的 探讨院前急救转运中使用一次性呼吸过滤器预防有创机械通气患者交叉感染的效果。方法 采用方便抽样法选取2023年1月1日至12月31日经太原市急救中心转运的40例有创机械通气患者为研究对象,按照随机数字法将其分为试验组和对照组各20例,在试验组患者使用的呼吸机送气端和患者端各安装1个一次性呼吸过滤器,对照组不安装。转运结束后,采集并比较两组患者呼吸机管路中的细菌菌落数及冷凝水量。结果 试验组患者呼吸机管路中的细菌菌落数[(2.56±0.85)cfu/cm2]低于对照组[(22.45±7.14)cfu/cm2]、冷凝水量[(0.56±0.33)ml]少于对照组[(5.24±3.16)ml],差异均有统计学意义(均P<0.01)。结论 一次性呼吸过滤器操作简单,使用方便,能有效降低有创机械通气患者呼吸机管路中的菌落数及冷凝水量,值得临床推广应用。

Objective To investigate the effect of using disposable respiratory filters in prehospital emergency transport to prevent cross-infection in patients with invasive mechanical ventilation.Methods Convenience sampling method was used to select 40 patients who were transported through Taiyuan Emergency Center and used invasive mechanical ventilation from January 1 to December 31,2021.They were randomly divided into the experimental group and the control group,with 20 cases in each group.One disposable respiratory filter was installed at the ventilator end and the patient end of the ventilator used by the patients in the experimental group,and not in the control group.The samples in the ventilator tubing were collected for bacterial culture after the transfer is completed,and the condensate volume was collected and measured,and the differences in colony number and condensate between the two groups were compared.Results The number of colonies in the observation group[(2.56±0.85)cfu/cm2] was significantly lower than that in the control group[(22.45±7.14)cfu/cm2],and the condensate volume [(0.56±0.33) ml] was lower than that of the control group [(5.24±3.16) ml].The differences were statistically significant (all P<0.05).Conclusions Disposable respiratory filters are easy to operate and convenient to use,and can effectively reduce the colonies and condensate volume in the ventilator tubing in patients with invasive mechanical ventilation.They are worthy of clinical promotion and application.
