目的 汉化和修订心力衰竭需求评估问卷（heart failure needs assessment questionnaire，HFNAQ），并验证其信效度。 方法 根据Brislin双向翻译原则汉化HFNAQ，经专家函询和预实验形成初稿,便利抽样选择520例慢性心力衰竭（chronic heart failure，CHF）出院患者进行信效度检验。 结果 HFNAQ汉化版共27个条目、4个维度（生理、心理、社会和自我管理），问卷总Cronbach’s α系数0.911，重测信度0.898；内容效度0.815~1.000。探索性因子分析提取4个公因子，累计方差贡献率58.24%。验证性因子分析显示模型拟合度可接受；各维度组合信度＞0.70，平均方差变异＞0.36。各维度平均方差变异平方根（0.636~0.768）大于该维度与其他维度的相关系数（0.571~0.654）。 结论 HFNAQ汉化版具有良好的信效度，可用于CHF患者出院后的需求评估。

Objective To translate and revise the Heart Failure Needs Assessment Questionnaire(HFNAQ),and test its reliability and validity. Methods The HFNAQ was translated into Chinese according to Brislin’s bidirectional translation principle.After expert consultation and pre-experiment revision,the first draft was formed and applied to 520 discharged patients with chronic heart failure(CHF) for reliability and validity test. Results The Chinese version of HFNAQ consisted of 27 items and 4 dimensions(physiological,psychological,social,and self-management).The questionnaire’s Cronbach’s α coefficient and 2-week test-retest reliability were 0.911 and 0.898 respectively.The content validity was 0.815~1.000.Exploratory factor analysis extracted 4 common factors,and the cumulative variance contribution rate was 58.24%.The results of confirmatory factor analysis showed that the overall model fit was acceptable;the composite reliability of each dimension was＞0.70,and the average variance extracted was＞0.36. The square root of average variance extracted of each dimension(0.636~0.768) is greater than the correlation coefficient between this dimension and other dimensions(0.571~0.654). Conclusions The Chinese version of HFNAQ has good reliability and validity,and can be used to assess the needs of CHF patients within one month after discharge.
