目的 对肿瘤患者放射性皮炎预防干预措施相关系统评价/Meta分析进行再评价,为预防癌症患者放射性皮炎循证护理实践提供证据支持。 方法 计算机检索循证卫生保健中心、Cochrane Library、CINAHL、OVID、Embase、PubMed、Web of Science及中国知网、万方、维普和中国生物医学文献数据库中关于防治放射性皮炎的系统评价/Meta分析,筛选文献后,采用系统评价方法学质量评价工具AMSTAR 2和推荐分级的GRADE系统对纳入文献进行方法学和证据质量的等级评价。 结果 共纳入25项研究,评价结果显示18篇质量等级为极低、5篇质量等级为低、2篇质量等级为中;对70条证据进行GRADE分级,结果显示极低级证据54条、低级证据12条、中级证据4条。 结论 放射性皮炎预防管理措施相关系统评价/Meta分析的方法学质量不高,证据质量等级较低;局部使用皮质类固醇和软聚硅酮敷料可降低放射性皮炎发生率和湿性脱皮发生率,缩短放射性皮肤损伤出现的时间;其他药物、皮肤护理等干预的效果有待高质量、大样本的研究进一步验证。

Objective To reevaluate the systematic review and meta-analysis of preventive interventions for radiation dermatitis(RD) in tumor patients,and to provide evidence support for evidence-based nursing practice for RD prevention in cancer patients. Methods Systematic reviews and meta-analysis on prevention and treatment of RD in JBI,Cochrane Library,CINAHL,OVID,Embase,PubMed,Web of Science,CNKI,Wanfang,VIP and Chinese biomedical literature database were searched.The methodological and evidence quality of the included literature was graded using AMSTAR 2 and the recommended GRADE system. Results A total of 25 studies were included,and the evaluation results showed that 18 studies were of extremely low quality,5 of low quality and 2 of medium quality.The 70 pieces of evidence were graded,and the results showed that 54 pieces were of extremely low level, 12 of low level and 4 of medium level. Conclusions The methodological quality of the systematic review and meta-analysis related to the preventive management measures of RD was not high,and the quality of evidence was low.Local application of corticosteroids and soft silicone dressings can reduce the incidence of RD and wet peeling,and shorten the duration of radioactive skin lesions.The effects of other drugs,skin care and other interventions need to be further verified by high-quality and large sample studies.
